Community Health Promotion Officer, End of Life and Palliative Care

We’re on the lookout for someone to play a vital role in implementing our End of Life and Palliative Care program.

What’s the project all about?

ACON is seeking a community health promotion officer to help to roll out a comprehensive ‘Taking Control’ End of Life and Palliative Care program, building on our existing work with community. In this role, you’ll be part of coordinating social spaces for people to talk about dying and death, online workshops, and bereavement workshops. This is an exciting opportunity to help LGBTQ+ communities across NSW to access information about their rights and options, and to have open and safe conversations about death and dying.

You’ll be supported by ACON’s Project Coordinator, Ageing, who also coordinates ACON’s LOVE Project, which aims to empower older lesbian, gay, bisexual, transgender and queer (LGBTQ) communities, including people living with HIV, to lead healthy, active and more social lives

Ok, so what’s the role?

The Community Health Promotion Officer, End of Life and Palliative Care is responsible for supporting the implementation of ACON’s ‘Taking Control’ End of Life and Palliative Care program for LGBTQ+ people in NSW.  You will:

  • Assist in the coordination, implementation, promotion and evaluation processes of ACON’s End of Life and Palliative Care project, including:
    • Planning events such as the ‘Death Dinner’ series, including coordinating guest speakers, run sheets and social activities
    • Organising and facilitating online palliative care workshops
    • Developing and co-delivering (with legal experts) legal workshops covering information about advanced care planning
    • Supporting the creation and roll out of a bereavement workshop to support LGBTQ+ people to work through grief and loss
    • Update and promote ACON’s existing Taking Control Toolkit- ACON’s LGBTQ+ Toolkit for Palliative Care and End of Life Decisions.

Who are we looking for?

  • Someone who’s comfortable having conversations about dying, death and grieving, and who is ready to support these conversations in community
  • Someone who’s confident in facilitation and building relationships with community members and stakeholders
  • Someone with an understanding of palliative and end of life concerns for sexuality and gender diverse communities
  • Someone who takes initiative and can work autonomously within a supportive team
  • Someone with experience in community event management, health promotion and/or community development

What’s the remuneration and working details?

Salary:                 $72,891.00 to $84, 357.00 (Gross per-annum, PLUS Superannuation and Leave Loading) – commensurate with skills, qualifications, and experience.

Location:            ACON’s Gadigal/Sydney Office, with flexible working options a possibility (away from the office)

Hours:                Full Time fixed term (70 hours per fortnight) until 30 June 2027

  • Salary Packaging up to $35,000 gross up value (i.e., currently up to $15,900 Expenses + $2,650 Meals, tax free).

Peer Facilitators – Sexual, Domestic and Family Violence

Ever wanted to make a positive difference as an LGBTQ+ person in your communities? Now is your chance!

We want to build a diverse pool of facilitators to deliver ACON workshops as LGBTQ+ peers. We are looking for people with some knowledge about sexual, domestic and family violence, and who are comfortable discussing these topics. We particularly encourage people with connections to the following communities and populations to apply:

    • Aboriginal and Torres Strait Islander people
    • trans people of all genders,
    • fetish and kink communities
    • sporting communities
    • party communities (eg people who are actively involved in planning club or party events),
    • people from migrant, refugee and culturally, linguistically diverse communities
    • GBQ+ men (cis and trans),
    • and people from regional locations.

We are on the lookout for some people to join the Sexual, Domestic and Family Violence team in the Health Equity and Harm Reduction Division to play a vital role in implementing our workshops.

These roles will support the implementation of ACON’s LGBTQ+ Sexual, Domestic and Family Violence (SDFV) Program for sexuality and gender diverse communities aimed at preventing and reducing the impact of SDFV experienced by LGBTQ+ people in NSW.

These are casual roles, where you will be offered shifts on an ad hoc basis (aka regular hours will not be offered). The workshop shifts offered will primarily be after business hours and weekends.

What workshops you say?

  • ACON’s healthy relationship workshops are a 4-week (one night a week), NSW based program for any LGBTQ+ person who wants to learn about healthy relationships. The workshops provide a safe and respectful environment for LGBTQ+ community members of all genders to share their experiences, understand factors that contribute to healthy relationships and learn new skills to create healthier relationships with their loved ones. The weeks content is all linked to four main themes: 1. Respect and Values, 2. Communication and Boundary Setting, 3. Sex and Consent, and 4. Conflict Resolution.
  • ACON’s strengthening LGBTQ+ community responses to disclosures of sexual violence workshops are a full day and NSW based. The workshops are for any LGBTQ community and allies to upskill their knowledge and capacity to build healthy consent focused environments and learn how to confidently respond to disclosures of sexual violence. These workshops are designed to provide a safe space for people to learn and understand more about sexual violence in LGBTQ+ communities including how to prevent it and how to respond to it.
  • Facilitators will be offered comprehensive training on workshop content

Ok, so what’s the role?

The Peer Facilitator – Sexual, Domestic and Family Violence is essentially co-facilitating workshops across NSW  to a range of audiences. You will:

  • Co-facilitate peer led healthy relationships and/or sexual violence disclosure workshops across NSW, including online.
  • Assist in the coordination, implementation, promotion and evaluation processes of ACON’s healthy relationships and sexual violence disclosure workshops.
  • Actively participate in debriefs and professional development where required
  • Perform other duties to assist with the work of the unit as requested by your supervisor (or designate).

Location:            Workshops will be held in various locations across NSW and online, as such we are looking both for casuals who can travel and facilitate in person workshops (Greater Sydney and beyond), and who can facilitate online workshops.

LGBTQ+ Group Worker

Ever wondered how to use your skills to help LGBTQ+ people have healthy and happy relationships?

  • We are on the lookout for someone to work across a number of ACON’s sexual, domestic and family violence (SDFV) programs, including facilitation and intakes for group programs/workshops, and being a family support worker for our behaviour change program as part of our SDFV team in the Health Equity and Harm Reduction Division.

What LGBTQ+ workshops/groups you say?

  • LGBTQ+ Survivor Groups is an 8-week intervention for people experiencing SDFV. It offers an important space for LGBTQ+ people to get support after an experience of violence in a safe and affirming group. This role sits in the SDFV team whose main goals are ending gender-based violence and supporting LGBTQ+ people who use or have experienced violence in NSW.

The LGBTQ+ Group Worker is essentially to run short-term solution-focused support via facilitating survivor groups for LGBTQ+ people in the area of SDFV. You will:

  • Co-facilitate LGBTQ+ groups both in person and online.
  • Conduct intakes for ACON groups such as the Survivor Group, and our Healthy Relationship workshops.
  • Be the family support worker for Proud Partners, ACON’s all gender behaviour change program.

  • Someone with an understanding of ACON’s communities and the unique complexities of working with LGBTQ+ clients who have experienced SDFV.

Peer Worker (Trans Mental Health and Wellbeing)

  • We are on the lookout for someone to join the Community Care team and play a vital role in our Trans Mental Health and Wellbeing program.
  • This position has come about through funding to support the mental health and wellbeing of trans and gender diverse people (including people questioning their gender).

Ok, so what’s the role?

The Peer Worker (Trans Mental Health and Wellbeing) uses lived experience to guide the provision of peer navigation, peer support, mentoring and advocacy to trans community members. The position sits within and works closely with the broader ACON Client Services team to deliver trauma-informed, wholistic person-centred support to clients.

You will be required to use your experience openly, appropriately and effectively to build professional relationships with the people you support. You will be providing support to people living throughout NSW face-to-face in the ACON office and via telehealth. Due to the nature of the role, you will be supporting individuals that are experiencing mental ill health, distress, suicidal ideation or suicidal crisis.

You will:

  • Provide peer navigation, peer support, mentoring and advocacy to trans and gender diverse people, including people who are questioning their gender, and trans people who are experiencing mental ill health, present in distress or are experiencing a suicidal crisis.
  • Use a strengths-based and person-led approach to help clients navigate to gender affirming care and support, and if required to support them to meet their recovery journey goals.
  • Support clients to strengthen their self-management skills through self-directed and shared learning experiences.
  • Work collaboratively with the broader clinical, non-clinical and health promotion teams within ACON to ensure that the broad psycho-social support needs of clients are being met.
  • Maintain timely client records by documenting work with clients in ACON’s electronic client record management system.
  • Collect required client data to enable reporting to funders and service evaluation, including demographics, client outcome and client experience data.
  • Conduct outreach support (as required) following ACON policies and procedures – this could include attending appointments or conducting home visits with clients to provide advocacy and support.

Who are we looking for?

Someone with the ability to reflect thoughtfully upon one’s personal lived experience of being trans or gender diverse, and an understanding of which elements may be common to other trans people, with the intention of providing peer navigation and peer support.

  • A person with lived experience of, and recovery from, psychological distress, mental ill health, and a willingness to share your story purposefully to support the mental health of others.
  • Someone with at least 6-months work in a peer position or volunteering with LGBTQ+ community members.
  • A person with a strong commitment to, and understanding of, trauma-informed recovery-based practice.
  • Someone who works independently and collaboratively in a team, reporting regularly to be supported.
  • A person with well-developed communication skills and demonstrated experience using MS Office.
  • Someone with proof of CoVid-19 Vaccination, in addition to other vaccination required to perform the role.

Our regional teams engage with LGBTQ+ people, community members and people living with HIV (PLHIV) in providing care coordination, peer support, counselling services, delivering health promotion campaigns, undertaking community development and peer education initiatives, supporting community groups and events, providing training and support to mainstream services, and providing access to the needle and syringe program.

Ok, so what’s the role?

The Team Leader, Community Health Promotion – Hunter is responsible for the day-to-day supervision of the Community Health Promotion Team, providing services across the Hunter, Central Coast, New England, and Western NSW regions. This includes the planning and delivery of ACON’s health promotion campaigns and community development responses with PLHIV, or people affected by HIV, and to sexuality and gender diverse people (LGBTQ+). There is a requirement for the person in this position to travel frequently within NSW and is based at ACON’s Newcastle office.

You will:

  • In consultation with the Regional Manager, lead and drive the successful delivery of health promotion services and activities to ensure key performance indicators (KPIs) and other deliverables are met.
  • Manage, support and lead Community Health Promotion Officers (CHPOs) and volunteer workers to achieve the goals and objectives of ACONs’ Business Plan and related strategies.
  • Assist in the development, implementation, monitoring and evaluation of health promotion activities and materials.
  • Assist with the development and implementation of strategies to adapt ACON services and programs to meet regional community needs.

What’s the remuneration and working details?: 

Salary:           $89,689 to $105,978 (Gross per-annum, PLUS Superannuation and Leave Loading) – commensurate with skills, qualifications, and experience.

Location:      ACON’s Awabakal / Hunter Office in Newcastle, with flexible working options a possibility (away from the office).

Hours:          Permanent Full-Time (70 hours per fortnight).

We offer flexible working arrangements and a generous array of entitlements often above the National Employment Standards (NES) including:

  • Salary Packaging up to $35,000 gross up value (i.e., currently up to $15,900 Expenses + $2,650 Meals, tax free).
  • Generous pro-rata leave entitlements – including up to an extra week of paid leave for full time employees to be taken during our Christmas and New Year’s shutdown period.
  • Employee Assistance Program (EAP) – access to a comprehensive counselling service free to employees.

Ok, so what’s the role?

The Associate Events Specialist works alongside ACON’s Pride Inclusion Programs’ Events & Social Media Producer to assist in the organisation and facilitation of a number of major events including the Australian LGBTQ+ Inclusion Awards, Pride in Sport Awards, Pride in Health + Wellbeing Awards, Pride in Practice Conference, Pride in Sport Summit and the programs presence at community events such as Mardi Gras’ Fair Day.

This role reports directly to the Associate Director, Operations, Events & Partnerships, Pride Inclusion Programs and requires:

  • Event planning and organisation.
  • Ticket registration, including managing guest lists.
  • Engaging and working with event suppliers, sponsors, and corporate professionals.
  • Sourcing event materials as needed.
  • Tracking and managing event budgets and expenses.
  • Internal and external communications including event promotion, event information and post-event feedback.
  • Attending and assisting on the day of the events, including bump-in and bump-out.
  • Assisting and co-ordinating with Audio Visual and Venue teams at events.
  • Liaising with Entertainers, MC’s and Speakers in the lead-up and during events.
  • Organising and assisting with merchandise sales.
  • Assisting with social media content creation and scheduling.

Who are we looking for?

Someone with:

  • Event management / coordination experience.
  • Knowledge and/or experience working with LGBTQ+ communities.
  • Experience in managing event budgets as well as sourcing and negotiating, and engaging suppliers.
  • Knowledge of, and experience using MS Excel, Word and PowerPoint.
  • Good oral and written communication skills for both internal/external stakeholders.
  • Ability to work independently and as part of a collegiate team; willing to assist across all areas during peak times.
  • High level organisation and efficiency, including the ability to manage multiple tasks and events at any one time.
  • Ability to maintain professional conduct when dealing with clients and stakeholders.
  • Ability to manage personal social media engagement in alignment with ACON’s policy and practices.

Some additional desirable attributes:

Someone with:

  • A current unrestricted Drivers Licence.
  • Social Media content creation and management experience.
  • Experience using Hootsuite, MailChimp and Survey Monkey.
  • Experience managing and updating WordPress
  • Experience organising and running Online Events using Zoom Events or similar platforms.
  • An understanding of and commitment to ACON’s communities.
